The boxed warnings and contraindications matter
Both labels carry a thyroid C-cell tumor boxed warning based on animal findings, with the human relevance described as unknown. They contraindicate use in people with a personal or family history of medullary thyroid carcinoma or MEN 2.[source][source]

Know when a symptom needs prompt attention
Current labeling describes serious risks including pancreatitis, gallbladder problems, severe gastrointestinal reactions, dehydration-related kidney injury and serious allergic reactions. Severe persistent abdominal pain or signs of a serious allergic reaction require prompt medical attention.[source][source]

Do not declare a safety winner from one percentage
The SURMOUNT-5 sponsor reported adverse-event discontinuation rates of 6.1% with tirzepatide and 8.0% with semaglutide, while stating that the study was not powered for a safety and tolerability comparison.[source]
Those figures are descriptive trial information. They do not prove that tirzepatide is safer for an individual, and they cannot be transferred to a compounded preparation or a telehealth service.
Safety is part of the continuing care arrangement
Ask who reviews symptoms, medication interactions and treatment changes. Ensure the prescriber knows about other diabetes medicines and planned procedures involving anesthesia. Zepbound labeling also includes specific guidance concerning oral hormonal contraception.[source]
